Apply for a NEW passport if any of the following apply:

• You are OVER 16 years of age and have never been issued a US passport, OR
• You are OVER 16 years of age and your prior passport was issued before your 16th birthday, OR
• You are OVER 16 years of age and your prior passport was issued more than 15 years ago.

Apply for a SECOND passport if:

• You are a frequent traveler who needs to obtain visas while traveling abroad or travel to politicially sensitive countries and desire a second passport for ease of use.
• You have a current, valid US Passport in your possession.

Apply for a PASSPORT NAME CHANGE AMENDMENT If:

• Your passport was issued less than a year ago AND
• You have legally changed your name since the passport was issued through a court order, marriage, divorce, etc.

NOTE: If you'd like to change the name in your passport and the passport was issued MORE than a year ago, then you'll need to RENEW your passport.

Apply for a LOST passport replacement if any of the following apply:

• Your lost passport was issued when you were age 16 or older.

Note: This process applies whether the lost passport was valid, expired, cancelled, or lost in another country - issued a temporary passport and received a letter stating you must apply with a DS11 application.

Apply for a CHILD/MINOR passport If:

• The child is currently under the age of 16.

NOTE: This includes minor's who have been issued a passport or not.

Apply for a Passport RENEWAL if your previously-issued passport meets ALL of the following:

• Your passport is in your possession and is not damaged AND
• Your passport was issued when you were age 16 or older AND
• Your passport was issued within the past 15 years AND
• Your passport was issued in your current name or you can legally document your name change.
